PURPOSE AND SCOPE:
The registered professional nurse (CAP RN 1) position is an entry level designation into the Clinical Advancement Program for Registered Nurses. The CAP RN 1 is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients.
As a member of the Chronic Kidney Disease (CKD) health care team, the CAP RN 1 participates in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and facility operations.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations.
• Performs all essential functions under the direction of the Clinical Manager (CM) and with the guidance from the Educator, Preceptor or a CAP RN 2, 3, 4 or 5 for the first 6 months of employment.
• Expected to complete requirements to advance to the next CAP level, for which they qualify, within 24 months from date of hire.
• Performs ongoing, systematic collection and analysis of patient data pre - during - post hemodialysis treatment for assigned patients and documents in the patient medical record, adjusts or modifies the treatment plan as indicated and notifies Team Leader, Charge Nurse, Clinical Manager or Physician as needed.
• Recognizes aspects and implications of patient status that vary from normal and reports to or collaborates with appropriate health team members for input.
• Assesses daily assigned patient care needs and collaborates with direct and ancillary patient care staff as needed.
• Directs and provides, in collaboration with direct and ancillary patient care staff, all aspects of the daily provision of safe and effective delivery of chronic hemodialysis therapy to assigned patients.
• Administers medications as prescribed or in accordance with approved algorithm(s), and documents appropriate medical justification and effectiveness.
• Ensures correct laboratory collection, processing and shipping procedures are performed and reschedules missed or insufficient laboratory collections.
• Ensures physician orders for assigned patients are entered and acknowledged in the Medical Record
• Initiates or assists with emergency response measures.
• Delegates appropriate tasks to direct patient care staff including but not limited to LVN/LPNs and Patient Care Technicians.
• Monitors patient care staff for appropriate techniques and adherence to FKC policy and procedures.
• Promotes infection control, equipment and environmental safety.
• Assesses, collaborates and documents patient/family's basic learning needs to provide initial and ongoing education to patients and family.
• Identifies expected outcomes, documents and updates the nursing assessment and plan of care for assigned patients through collaboration with the Interdisciplinary Team.
• Ensures patient awareness related to transplant and treatment modality options.
• Makes referrals to Social Worker and Registered Dietitian as appropriate for individual patient education and counseling.
• Checks availability of adequate emergency equipment and supplies as assigned.
• Assists with medication tracking and inventory as assigned.
• Serves as a resource for health care team, participates in staff training and orientation of new staff as assigned.
• Participates in all required staff meetings as scheduled.
• Supports and promotes the FMCNA's mission, vision, values, and customer service philosophy.
• Performs all other duties as assigned by supervisor.
PHYSICAL DEMANDS AND WORKING CONDITIONS:
• The physical demands and work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
• The position provides direct patient care that regularly involves heavy lifting and moving of patients, and assisting with ambulation. Equipment aids and/or coworkers may provide assistance. This position requires frequent, prolonged periods of standing and the employee must be able to bend over. The employee may occasionally be required to move, with assistance, machines and equipment of up to 200 lbs., and may lift chemical and water solutions of up to 30 lbs. as high as 5 feet. The work environment is characteristic of a health care facility with air temperature control and moderate noise levels. May be exposed to infectious and contagious diseases/materials.
• Day to day work includes desk work, computer work and interaction with patients, facility/hospital staff and physicians. The position may require travel to training sites or other facilities.
• May be asked to provide essential functions of this position in other locations with the same physical demands and working conditions as described above.
SUPERVISION:
• Assigned oversight of Patient Care Technicians/LPNs as a Team Leader or designated Nurse in charge, after the following:
• Successful completion of all FKC education and training requirements for new employees plus
• Must have a minimum of 9 months experience as an RN plus
• Successful completion of 3 months experience in chronic hemodialysis.
EDUCATION:
• G raduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Must meet the practice requirements in the state in which he or she is employed.
EXPERIENCE AND REQUIRED SKILLS:
• Entry level for RNs with less than 2 years of nephrology nursing experience within the last two years or for RNs with a BSN and less than 1 year of nephrology nursing experience within the last two years.
• Minimum of 9 months experience as a Registered Nurse (preferred).
• Chronic hemodialysis experience (preferred).
• Successful completion of FKC Education and Training requirements for new employees.
• Successful completion and maintenance of CPR BLS Certification.

• Follows through and documents all findings, corrections or counseling for all nursing incidents, medication errors, patient complaints and forwards to the Director of Nursing for disposition.
• Identifies potential problems, establishes priorities, initiates effective problem solving based on clinical knowledge and informs Director of Nursing of proposed corrective actions.
• Ensures Director of Nursing is informed of any concerns raised by the nursing staff regarding the questionable lack of appropriate medical intervention for the realization of established patient goals.
• Maintains composure in stressful situations, aware of own limitations and seeks guidance as necessary from the Director of Nursing.
• Assumes administrative and clinical call on a scheduled basis when requested.

• Assists the DON as needed to ensure an accurate account of monthly staffing and statistics as required by contract.
• Assists in the orientation and on-the-job training for all new employees assigned to the unit.
• Ensures employees are assigned duties and responsibilities to the level of their expertise, licensure or certification.
• As delegated by the Director of Nursing, annually evaluates performance of supervised staff according to established policy and procedure.
• As assigned conducts staff meetings for all supervised health care providers to identify problems, propose solutions, set goals.
• Assists in the collection of monthly statistics as assigned.
• Reviews, monitors, and maintains perpetual inventory of all contraband and tool counts; investigates and reconciles any discrepancies and reports findings to the Director of Nursing.
• Cleanliness, maintenance, sanitation and appearance of unit and appropriately notifies security of any infractions needing correction or attention.
• Coordinates patient care with other departments, to include but not limited to, intake admission, transfers, infirmary admissions, off-site hospital admission, in-house referrals, off-site referrals, periodic and annual physicals, mandatory release, parole, and patient teaching.
• Assists the DON in ensuring that appropriate levels of stock for pharmacy, medical supplies and laboratory supplies are maintained. Provides for preventive maintenance for all medical equipment as appropriate, and initiates procedure for repair as indicated.
• Assists in the maintenance of a professional atmosphere on the nursing unit as well as a high standard of patient care.
• Sets good example for the staff through regular attendance, arriving promptly on time for duty, being appropriately dressed well groomed, adhering to the dress code, and displaying the identification badge appropriately and being neat and well groomed.
• Attends monthly institutional staff meetings as directed by the DON or Administrator.
• Other duties as assigned by the Health Service Administrator or Director of Nursing.
• Must be able to obtain and maintain security clearance.
• Must be able to apply principles of critical thinking to a variety of practical and emergent situations and accurately follow standardized procedures that may call for deviations.

Duties
The ICU is a fast paced critical care environment. Responsibilities include, but are not limited to:
caring for patients with multiple complex surgical and medical conditions, mechanical ventilation, intra-aortic balloon pump, continuous veno-venous hemodialysis (CVVHD), etc. The incumbents will also be required to provide coverage for all critical care areas as needed. Per local policy, the RN is responsible for maintaining Basic Life Support (BLS) and Advance Cardiac Life Support (ACLS) certifications. If not ACLS certified within 6 months of assignment, may be reassigned to a different unit.
Combination of 12 hour and 8-hour shifts
Day Shift
12-hour shift: 7:30 am to 8:00 pm
8-hour shift: 7:30 am to 4:00 pm

Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical inpatient lifts are provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. Additional Responsibilities:
• Completes an initial assessment of patient and family to determine their needs. Provides a complete physical assessment and history of current and previous illnesses.
• Provides professional nursing care by utilizing all elements of the nursing process.
• Assesses and evaluates patient’s status by:
• writing and initiating Plan of Care.
• Regularly re-evaluating patient / family / caregiver needs.
• Revising the Plan of Care as necessary.
• Initiates the Plan of Care and makes necessary revisions as patient status and needs change.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician’s Plan of Care.
• Counsels the patient and family in meeting nursing and related needs.
• Provides health care instructions to the patient as appropriate per assessment and plan.
• Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency.
Skills Needed
• IV infusion via PICC line or Mediports, knowledge in infusing ABT, TPN, Biologics, Inotropes, and IVIG, Strong peripheral IV insertion skill preferred.
Position Qualifications
• A graduate of an accredited school of nursing. One or two years of recent acute experience in an institutional setting.
• Current licensure in State, CPR certification, and valid driver’s license.
• RNs are responsible for supervising home health aides and LPN’s.
• Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, nursing skills per competency checklist.
• Prolonged or considerable walking or standing. Able to lift, position, and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, and/or crouching. Visual acuity and hearing to perform required nursing skills
